Elden Ring Millicent Questline: Every Location, Choice, and Reward
Follow Millicent's Elden Ring questline from Gowry and the Unalloyed Gold Needle to every location, the final choice, and both reward paths.
Spoiler warning: this guide names the final choice, character outcomes, and every reward immediately so you can avoid locking yourself out of the branch you want.
Millicent’s quest starts with Gowry in Caelid and ends with an irreversible choice in Elphael. Choose the gold summon sign to assist Millicent and receive the Rotten Winged Sword Insignia, then recover the Unalloyed Gold Needle for the Miquella’s Needle route. Choose the red sign to challenge her and receive Millicent’s Prosthesis instead; that ends the needle path for this journey.
This walkthrough was last verified on September 9, 2026 for Elden Ring App Ver. 1.17 / Regulation Ver. 1.17 on PlayStation 4, PlayStation 5, Xbox One, Xbox Series X|S, and Steam. Bandai Namco’s official 1.17 patch notes do not list a change to Millicent’s quest. The route below is corroborated against the current published walkthroughs cited in the relevant sections; it is not presented as first-hand testing.
Should you assist or challenge Millicent?
Assist Millicent if you want the Rotten Winged Sword Insignia or need Miquella’s Needle; challenge her if you specifically want Millicent’s Prosthesis. The completed summon-sign choice determines this playthrough’s reward branch.

The talismans support different decisions rather than one universal “best” choice. Rotten Winged Sword Insignia increases attack power during successive attacks. Millicent’s Prosthesis also rewards successive attacks and adds 5 Dexterity. If removing the Frenzied Flame lock may matter later, the assist branch is required because only that route returns the needle.
Do not confuse the three similarly named quest items: the Prosthesis-Wearer Heirloom is Millicent’s early thank-you talisman, the Valkyrie’s Prosthesis is the arm you bring her in Altus Plateau, and Millicent’s Prosthesis is the final talisman from challenging her.
What are all the rewards in Millicent’s quest?
The complete route can award the Prosthesis-Wearer Heirloom before the branch, one of two branch talismans, and—only after assisting—two later items from Malenia’s bloom. The Valkyrie’s Prosthesis is a key item you hand over, not a reward you keep.
- Prosthesis-Wearer Heirloom: received at the Church of the Plague after the repaired needle cures Millicent; it adds 5 Dexterity.
- Rotten Winged Sword Insignia: received after winning the gold-sign assist fight; it increases attack power with successive attacks.
- Millicent’s Prosthesis: received from the red-sign challenge branch; it adds 5 Dexterity and increases attack power with successive attacks.
- Unalloyed Gold Needle: recovered after the assist branch and Millicent’s final dialogue; it remains a quest item for the next exchange.
- Miquella’s Needle and Somber Ancient Dragon Smithing Stone: received together by returning the Unalloyed Gold Needle to the bloom after defeating Malenia.
- Flock’s Canvas Talisman: available from Gowry at the end of either branch; the exact collection step differs by choice.
- Gowry’s Bell Bearing: an optional assist-route drop if you kill Gowry after he begins mourning Millicent.
The early heirloom does not prevent either final choice. The lockout applies when you complete the gold or red summon encounter, so reaching the signs is the last safe point to compare the two branch rewards. On the challenge route, return to Gowry’s Shack and collect the Flock’s Canvas Talisman he leaves. On the assist route, recover Millicent’s needle first, then return to the distraught Gowry. Killing him at this finished state drops Flock’s Canvas Talisman and Gowry’s Bell Bearing. Killing an NPC is destructive, so do not do it before finishing Millicent’s dialogue and recovering the needle.
What do you need before starting Millicent’s quest?
You need access to eastern Caelid and the ability to defeat Commander O’Neil in the Swamp of Aeonia. Finishing the full route later requires Altus Plateau, the Mountaintops of the Giants, both halves of the Haligtree Secret Medallion, and access to Elphael in Miquella’s Haligtree.
- Talk until dialogue repeats at every Millicent and Gowry stop.
- Rest at a Site of Grace or reload the area after handing over an item or defeating a required enemy.
- Keep the Unalloyed Gold Needle and Valkyrie’s Prosthesis for their quest steps.
- Expect late-game spoilers from the Haligtree section onward.

How do you start the Millicent quest in Caelid?
Speak to Gowry at Gowry’s Shack south of Sellia, defeat Commander O’Neil for the Unalloyed Gold Needle, and bring it back to Gowry for repair. You can then open the route through Sellia and give the repaired needle to Millicent at the Church of the Plague.
1. Speak to Gowry at Gowry’s Shack
Find the shack on the road south of Sellia, Town of Sorcery. Gowry asks you to recover an Unalloyed Gold Needle for a sick woman at the Church of the Plague. Exhaust his dialogue before leaving.
2. Defeat Commander O’Neil
Commander O’Neil is in the Swamp of Aeonia, west of Gowry’s Shack. Defeat him and collect the Unalloyed Gold Needle. Return it to Gowry. Rest or reload the area, then speak to him again to receive the repaired needle.
3. Open Sellia and reach the Church of the Plague
Light the three braziers on Sellia’s towers to remove the magical barriers and open the uphill route behind the town. Follow that route to the Church of the Plague. Two Kindred of Rot guard the approach, so clear them before holding a conversation inside if they are pursuing you.
4. Cure Millicent and collect the first talisman
Give the repaired needle to Millicent against the church wall. Rest or reload, then speak to her until she gives you the Prosthesis-Wearer Heirloom. This talisman adds 5 Dexterity, but it is not Millicent’s Prosthesis from the final branch.
Reload again after she leaves, then return to Gowry’s Shack and speak to Millicent there. Exhaust her dialogue so her journey can advance. These early steps follow the ordered state changes documented in IGN’s Millicent walkthrough.
Where is Millicent in Altus Plateau?
Millicent first waits just north of the Erdtree-Gazing Hill Site of Grace. She will not continue until you bring her the Valkyrie’s Prosthesis from the Shaded Castle.
5. Meet Millicent at Erdtree-Gazing Hill
From the Site of Grace, walk north toward the cliff and speak to her beside the rocks. Exhaust the dialogue. If she remains here afterward, that is expected until you deliver the prosthesis.
6. Get the Valkyrie’s Prosthesis from the Shaded Castle
Enter the Shaded Castle in northern Altus Plateau. The Valkyrie’s Prosthesis is in a chest in an upper room watched by a Cleanrot Knight. IGN’s route approaches from the castle’s northeastern outer side, crosses the rooftops, and follows the rampart to the guarded room; the Shaded Castle Inner Gate grace is nearby but does not provide a direct path into that room.
Return to Erdtree-Gazing Hill and give the Valkyrie’s Prosthesis to Millicent. Rest or reload after exhausting her response.
7. Speak to her at Dominula, Windmill Village
Go to Dominula in northeastern Altus Plateau and defeat the Godskin Apostle at the top of the village. Millicent appears nearby after the boss is dead and she has received the Valkyrie’s Prosthesis. Speak to her beside the Windmill Heights Site of Grace.
Where does Millicent go after Altus Plateau?
Her next stops are Ancient Snow Valley Ruins in the Mountaintops of the Giants and the Prayer Room in Elphael. There is no new item handoff at either stop; exhaust her dialogue to record each state.
8. Ancient Snow Valley Ruins
Find Millicent beside the Ancient Snow Valley Ruins Site of Grace. Talk until her lines repeat. She explains that she is heading toward Miquella’s Haligtree.
9. Prayer Room in Elphael
Use the completed Haligtree Secret Medallion to reach the Consecrated Snowfield. In Ordina, enter the evergaol and light all four figures to unseal the portal to Miquella’s Haligtree; the Haligtree access walkthrough shows this mandatory gate. Continue through the Haligtree into Elphael. Millicent stands beside the Prayer Room Site of Grace. Exhaust her dialogue before moving toward the Drainage Channel.
How do you make Millicent’s final summon signs appear?
Kill the Putrid Tree Spirit in the scarlet-rot pool near the Drainage Channel route, then rest or reload. The gold and red summon signs appear on the bank above the pool after that field boss is dead.
10. Return from the Drainage Channel to the rot pool
From the Drainage Channel side, follow the roots back toward the elevated rot pool. Entering the pool triggers the Putrid Tree Spirit, the scarlet-rot Elphael variant of the Tree Spirit enemy. Defeat it, leave the pool, and reload the area. If the signs are missing, confirm that the spirit is dead, rest at the Drainage Channel grace, and return to the bank.
11. Choose the gold or red sign
The gold sign summons you to assist Millicent against her four sisters. The red sign sends you to challenge Millicent. IGN and an independent step-by-step route agree on the sign colors and their opposing talisman rewards.
- Select gold and win the fight to receive the Rotten Winged Sword Insignia.
- Select red and defeat Millicent to receive Millicent’s Prosthesis and end her needle route.
There is no confirmation screen explaining the later needle consequence. Decide before interacting with a sign.
How do you finish the assist-Millicent branch?
After winning against the four sisters, return to your world, find Millicent beside the pool, and exhaust her dialogue. Reload once more and collect the Unalloyed Gold Needle where she was sitting.
- Approach using the bank or roots; do not land on Millicent or attack near her.
- Talk until her dialogue repeats.
- Rest or reload the area.
- Return to her position and take the Unalloyed Gold Needle.
- Defeat Malenia, Blade of Miquella.
- Use the needle on the scarlet bloom left in Malenia’s arena to receive Miquella’s Needle and a Somber Ancient Dragon Smithing Stone.
Miquella’s Needle does not remove the Frenzied Flame merely by sitting in the inventory. Use it inside Dragonlord Placidusax’s arena to reverse the Frenzied Flame lock and reopen other ending choices.
Why is Millicent missing or not moving?
A missing Millicent usually means the previous dialogue, item handoff, boss, or reload trigger is incomplete. Check the last confirmed location rather than jumping randomly between every stop.

Does Millicent’s quest affect an Elden Ring ending?
The quest does not choose an ending by itself. Its assist branch provides the route to Miquella’s Needle, which can remove the Frenzied Flame lock when used in Dragonlord Placidusax’s arena.
Can you get both Millicent talismans in one playthrough?
Not through the normal completed quest branches. Assisting awards Rotten Winged Sword Insignia; challenging awards Millicent’s Prosthesis. Plan to obtain the other in New Game Plus or another journey rather than relying on old kill-after-assist exploits.
Can you complete Millicent’s quest without Shadow of the Erdtree?
Yes. The entire quest, including Elphael, Malenia, and Miquella’s Needle, is base-game content. The expansion is not a prerequisite.
