A totem of undying comes only from evokers, and evokers always drop exactly one totem on death. You can find evokers in woodland mansions and in raids on Normal or Hard difficulty. Held in your off-hand or main hand, the totem revives you from otherwise fatal damage.
Where to find evokers
Woodland mansions are one source. Buy a Woodland Mansion Map from a Master-level cartographer villager for 14 emeralds plus a compass, and the map marks the nearest mansion. Evokers there spawn only during world generation, in particular rooms on floors 2 and 3, and they do not respawn.
Raids are the other source. Evokers spawn in raids only on Normal or Hard difficulty. In Java Edition, evokers start spawning at wave 5, and up to five evokers can spawn on Hard. Trigger a raid by gaining Bad Omen from killing a raid captain or drinking an ominous bottle and entering a village. Bad Omen converts to Raid Omen, and the raid begins when Raid Omen expires.
How to use the totem
The totem must be held in the off-hand or main hand. It does not work from the hotbar unless that slot is selected. A totem is single-use, restores 1 HP, and removes all existing status effects when activated.

Fighting evokers
Evokers have 24 HP. Their fang attack deals 6 damage on Normal, 4 on Easy, and 9 on Hard in Java Edition. Fang damage ignores armor but is reduced by the Protection enchantment. Where it goes wrong
- You take fatal damage while not holding the totem in the off-hand or selected main hand.
- You expect the totem to save you from void damage or the /kill command; it cannot.
- You clear a woodland mansion and expect evokers to respawn; they do not.
- You run a raid on Easy difficulty; evokers never spawn there.
- You hope Looting will increase the drop; evokers always drop exactly one totem.
